1. Service Request
A request must be submitted to the specified postal or email address, or in person, in accordance with the requirements of Article 6 of the Public Entertainment and Festive Events Safety Law.
- In the request for organizing a public event, the event organizer must provide the following information:
1) event organizer (for individuals — name, surname, personal code, and residence address; for legal entities — name of the legal entity, registration number, and legal address);
2) person responsible for technical safety (for individuals — name, surname, personal code, and residence address; for legal entities — name of the legal entity, registration number, and legal address);
3) person responsible for public order and safety (name, surname, personal code, and residence address);
4) order maintainers (for individuals — name, surname, personal code, and residence address; for legal entities — name of the legal entity, registration number, and legal address);
5) type and purpose of the event;
6) event location, date, start time, and planned end time;
7) estimated number of attendees and participants;
8) hazardous equipment to be used at the event;
9) necessary support from state and municipal authorities for the unobstructed and safe conduct of the event.
Attach to the request:
1)copies of contracts concluded by the event organizer with order maintainers, persons responsible for the technical safety of the public event, as well as for public order and safety, presenting the originals;
2) a detailed event plan;
3) written consent from the owner of the event location for its organization if they are not the event organizer.
If hazardous equipment will be present or used at the public event location, the event organizer must attach the required permits for the use of such equipment from the owner, as stipulated in the regulations, along with copies of other documents necessary for the operation of this equipment, presenting the originals.
When submitting the request, the event organizer must present an identification document, while the representative of a legal entity must also present documents that confirm their authority to represent the respective legal entity.
If the public event location is situated within the administrative territories of multiple municipalities, the request must be submitted to all relevant municipalities. The event organizer sends a copy of the request to the appropriate State Police authority.
