1. Service requests In order to assess conformity with the status of a deprived\/low-income household, one of the household persons shall turn to the social service office of the local government, present a personal identification document and submit: - an application signed by the adult persons of the household; - a statement of the employer regarding the remuneration for the last three calendar months for each employee in the household; - a statement regarding income from economic activity for the last three calendar months for each performer of economic activity in the household; - statements of accounts of all credit institutions or postal settlement system accounts of persons in the household for the last three calendar months; - documents certifying irregular income or other income and payments received for the 12 calendar month period prior to submission of the application; - documents for the calculation of the housing benefit, which certify expenditure related to the use of the dwelling, as well as documents certifying the use of the dwelling (for example, a rental contract) shall be presented; - other documents, if it is necessary for taking a decision regarding determination of the status of a deprived household. 2. Decision making After receipt of the referred to documents and preparation of the declaration the social service office shall evaluate the material resources of the household, calculate basic social assistance benefits (guaranteed minimum income benefit and housing benefit) and take a decision to grant social assistance benefits or to refuse to grant social assistance, as well as take a decision regarding conformity of the household with the status or refusal of a deprived or low-income household. 3. Receipt of services The conformity of a household with the status of a deprived or low-income household shall be certified by a statement issued by the social service, in which the given names, surnames, personal identity numbers, birth data, as well as the term of validity of the statement and the regulatory enactment on the basis of which the relevant status has been determined are indicated.

The decision of the local government social service office may be contested in the local government city Council. A decision taken by a local government city or municipality Council may be appealed to a court in accordance with the procedures laid down in the Administrative Procedure Law.
